Immunization Waivers

Effective January 1, 2015, the State of Michigan changed the way that nonmedical immunization waivers can be obtained. Any parent/guardian who wants to claim a non-medical waiver will need to receive education regarding the benefits of vaccinations and the risk of disease from a county health department before obtaining the certified non-medical waiver form through the local Health Department.

Below are some useful links that will provide you with more information about the immunization waiver changes.   You may schedule an appointment for Immunization Waiver Education by calling 1-800-848-5533.

Frequently Asked Questions - Immunization Waivers

If your child has a medical reason for not receiving a vaccine, a physician must sign the State of Michigan Medical Contradiction form. This form is available at your doctor's office, not the county Health Department. The completed Immunization Waiver (medical or non-medical) is then turned into your child's school.
